Behavioral Health Navigator helping resident

Unsure of where to start with behavioral health services? We can help! Every Tuesday from 8 am to noon our Behavioral Health Navigators are at Dedicated Senior Medical Center, 4715 S Florida Ave, Suite 200, Lakeland, FL 33813.

Behavioral Health Navigation is designed to improve access to, and knowledge of, mental health and substance use disorder treatment services available in Polk County. The program is available for Polk County residents of all ages who have behavioral health needs and would benefit from help navigating the care system. Our navigators can provide information about what services are available, assist with referrals to care, and even help individuals apply for benefits. These navigation services are provided at no cost. We hope to see you soon!
