Adult Case Management Services (ACM)
This program assesses people with mental illnesses for service needs and links them to the needed community resources such as social security, Medicaid/Medicare and food assistance. ACM also helps people plan short and long-term recovery goals while monitoring progress on reaching their recovery goals. ACM also serves as an advocate in the individual’s recovery. Referral required.

Club Success
Locations in Lakeland and Haines City
Club Success is a recovery program for people with mental illness and attendees are called members rather than patients or clients. Part of a program certified by Clubhouse International, which includes more than 370 Clubhouses in 32 countries throughout the world, the Clubhouse model is designed to mirror a mainstream day in the workplace.
Members learn responsibility, develop self-esteem, and learn to interact professionally. Members are responsible for using their abilities to help others and build relationships while working closely with staff and colleagues. The clubhouse provides members with transitional employment opportunities through local community partners, guidance to secure housing, and/or continue their education. It also promotes members’ ongoing psychiatric care.

Forensic Services
This program provides a wide range of services to individuals involved in the criminal justice system, including community monitoring and resource linkage, for adult individuals who have been deemed incompetent to proceed or not guilty by reason of insanity. Includes the Conditional Release Program and the Community Restoration Program. Referral required.

Psychosocial Rehabilitation Services
Psychosocial Rehabilitation (PSR) Services offers structured psychoeducation and support groups for adults with a primary diagnosis of Schizophrenia, Bipolar, Anxiety or Depression. Services assist participants with learning skills related to symptom management, socialization, independent living, life management and educational/vocational endeavors. Participants choose the groups in line with their personal recovery goals. A different topic is taught every day in small group settings, which allows for more one-on-one time. Referral required.
