Every Monday from 9 am to 1 pm our Navigators are at Specialty Care Clinic, 1255 Brice Blvd, Bartow, FL 33830 to help Polk residents. Whether that’s answering questions about local resources or completing referrals to care, we are there for you! Stop by and see us.
Behavioral Health Navigation is designed to improve access to, and knowledge of, mental health and substance use disorder treatment services available in Polk County. The program is available for Polk County residents of all ages who have behavioral health needs and would benefit from help navigating the care system. Our navigators can provide information about what services are available, assist with referrals to care, and even help individuals apply for benefits. These navigation services are provided at no cost.
