What is Behavioral Health Navigation?

Behavioral Health Navigation (BHN) is a program designed to improve access to, and knowledge of, mental health and substance use disorder treatment services available in Polk County.

Who does the program help?

Behavioral Health Navigation (BHN) is available for Polk County residents of all ages who have behavioral health needs and would benefit from help navigating the care system.

How can BHN help me?

Our navigators will share information about what services are available, assist you with referrals to care, and even help you apply for benefits. Our navigators are also here to help you complete system-based requirements for the appropriate level of care. Our services are provided at no cost.

Why BHN?

We have built a resource network and will ensure that services are matched specifically to your identified needs. Our goal is to eliminate barriers, help you access needed care, understand available resources, overcome any obstacles and link you to the appropriate level of care and support.

What does the process involve?

Our navigator will work with you to complete an assessment, then create a personal plan to help you navigate through the behavioral health systems and resources.

If you have a child who has been having difficulty at school but are unsure what kind of support or services are available to assist.
A Behavioral Health Navigator can talk with you about your specific situation and provide you with a list of options for services that could help either with support at school, mental health services or both.
If things have not been great at work or home, and you are feeling overwhelmed by the pressures of everyday tasks and are not sure what services are out there for you.
A Behavioral Health Navigator can provide information for support groups and professional supports to help you feel better.
Maybe things are good right now, but your financial situation changed, and you are unsure about health coverage options and how to get health coverage.
A Behavioral Health Navigator can provide you with information for available options and help with the application process.
Perhaps you already have health coverage, but your doctor is now out of network, and you are not sure what to do.
A Behavioral Health Navigator can provide guidance with the process of establishing a new provider that works with your current health care plan.

Behavioral Health Navigation in the Community

If you’re out and about, you can stop by one of our recurring outreach booths to ask questions, find out about local resources, and more.

Every other Monday 9 am to 1 pm
