New PRC Program Exceeds Goal
We are proud to share Peace River Center’s Care Coordination program has officially wrapped up its first year of service.
Care Coordination is a SAMHSA-funded program serving Polk, Hardee, and Highlands counties. This grant aims to help adults diagnosed with severe and persistent mental illness, co-occurring disorders, and children diagnosed with severe emotional disturbance. Clients who have experienced a Baker Act and have mental health symptoms impacting their daily functioning also qualify. The Care Coordination program is a short-term linkage to community-based resources. Care Coordination also has an SSI/SSDI Outreach, Access, and Recovery (SOAR) Specialist who can assist qualifying clients with the SSI/SSDI application process and other insurance benefit options.
Their goal for the first year was to serve 275 clients, and they managed to provide services to 309 clients. What an accomplishment!
“The team worked together, utilizing each other’s strengths and unique experiences to ensure clients were linked to services and resources that would help them thrive in the community we all love,” said Cassie Carver, LMHC, CCTP, CTMH, PRC’s Director of Outpatient Therapy. Cassie said the Care Coordination team’s hard work and positive impact on our community are truly something to celebrate.
“It’s not easy to walk into a brand new program as a supervisor and hit the ground running, but that is what Simone did. No matter what curve ball was thrown her way, she would respond, “okay, gotcha,” and she’d get to work while simultaneously training all new staff and helping anyone who asked,” said Cassie. “Simone’s hands-on approach has led her team through a very successful first year.”
